Brain Abscess
A brain abscess is a local accumulation of pus within the brain tissue that presents as a lesion or enlarging mass and is potentially leading to profound neurological deficits or even death. Headache, fever, vomiting, seizures, confusion, drowsiness, and focal weakness or paralysis are common symptoms. Diagnosis is achieved with CT scans, usually supplemented by MRI to better define the size and nature of the abscess. Causes most commonly involve spread of infection from distant sites or contiguous locations like the sinuses or nose and entry or propagation by trauma associated with dental and skeletal infections. Treatment is typically a strategic combination of surgical drainage—relieving pressure and facilitating healing—and selective antibiotic or antifungal treatment to destroy the etiologic agent. Corticosteroids such as dexamethasone can be used to reduce inflammation. Follow-up is close observation of the course of the abscess, with additional antibiotics or surgery being held in reserve to be used if initial therapy is insufficient. Early diagnosis, imaging, and treatment are important to reducing the neurological effects of a brain abscess.
